RN or LPN Per Diem role

  • Are you a compassionate and dedicated nurse seeking a rewarding career? The Converse Home, a warm and welcoming assisted living community in Burlington, VT, is seeking a per diem nurse to cover open shifts across both our memory care and traditional communities. 
  • Key Responsibilities:
  • Provide high-quality nursing care with compassion and professionalism.
  • Administer medications and treatments in accordance with care plans and physician orders.
  • Monitor residents’ health status and promptly report changes to the healthcare team.
  • Collaborate with interdisciplinary team members to develop and implement individualized care plans.
  • Educate and support residents and their families.
  • Maintain accurate, timely, and thorough medical documentation. 
  • Qualifications:
  • Must have a Current RN/LPN license in the state of Vermont.
  • Experience in assisted living, long-term care, or a related setting preferred.
  • Strong organizational and time-management skills.
  • Excellent communication and interpersonal skills.
  • A team-oriented, patient-centered, and compassionate approach to care.

As a Resident Care Assistant, you will play a key role in supporting the health, comfort, and well-being of our residents. This position includes assisting with daily care needs, administering medications, offering meaningful companionship, and helping maintain a supportive, respectful community environment. This role is ideal for someone who is passionate about making a positive impact in the lives of older adults. We are looking to fill full time and part-time or per diem openings. We currently only have part-time spots available

  • Key Responsibilities:
  • Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility.
  • Administer medications accurately and safely in accordance with state regulations and community policies.
  • Provide compassionate, resident-centered care that supports dignity and independence.
  • Observe, document, and report changes in residents’ health or behavior to nursing staff.
  • Help maintain a clean, safe, and comfortable living environment.
  • Build positive relationships and communicate effectively with residents, families, and team members.
  • Follow all safety protocols, care standards, and organizational procedures.
  • Qualifications:
  • High school diploma or equivalent required.
  • Previous caregiving or healthcare experience strongly preferred.
  • Experience with medication administration required; LNA certification is a plus.
  • Strong communication and interpersonal skills.
  • Ability to use a computer for documentation.
  • Compassionate, patient, and dependable.
  • Ability to work both independently and collaboratively.
  • Must successfully pass a background check.
