Clinical Lead (RN/LPN) Memory Care Community
Employment Type: Full-Time, 40 Hours per Week (32 Hours as Lead and Charge Nurse, 8 Hours Floating for Meetings/Paperwork)
Schedule: Includes Every Other Weekend
Position Overview: As the Clinical Lead of our Memory Care Community, you will play a vital role in ensuring the highest quality of care for our residents. You will manage a small community of 16 residents, supervise the memory care nursing staff, and work closely with the Director of Nursing (DON) and Assistant Director of Nursing (ADON) within the scope of care. This position requires exceptional communication skills, attention to detail, and the ability to maintain a cohesive team environment across all shifts and departments.
Key Responsibilities:
- Oversee the clinical operations of the Memory Care Community, ensuring residents receive the best care possible.
- Manage and supervise the memory care nursing staff, fostering a collaborative and supportive team environment.
- Collaborate with families, healthcare providers, and outside agencies to coordinate comprehensive care plans for residents.
- Maintain accurate and up-to-date documentation, including care plans, medical records, and reports.
- Conduct regular meetings with staff, families, and external partners to ensure effective communication and continuity of care.
- Monitor and evaluate the performance of nursing staff, providing guidance, training, and professional development opportunities.
- Ensure compliance with all relevant regulations, policies, and procedures.
Qualifications:
- Registered Nurse (RN/LPN) with a valid license in the state of Vermont.
- Experience working with the elderly, particularly in a memory care setting.
- Strong leadership and supervisory skills.
- Exceptional communication and interpersonal abilities.
- Attention to detail and a commitment to delivering high-quality care.
- Ability to work effectively under the DON and ADON within the scope of care.
- Proven ability to maintain a cohesive team environment across all shifts and departments.

RCA
As a Resident Care Assistant, you will play a vital role in enhancing the quality of life for our residents. You will assist with daily living activities, provide companionship, and ensure that our residents receive the highest level of care and respect. This is an excellent opportunity for individuals who are passionate about making a difference in the lives of others.
We are currently looking for PT RCAs to work in both our traditional and memory care communities:
Some Shifts available:
- 6:30am-1pm.
- 4-10pm
Key Responsibilities:
- Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and mobility.
- Provide compassionate and respectful care to residents while maintaining their dignity and independence.
- Monitor and document residents’ health and well-being, reporting any changes to the nursing staff.
- Help maintain a clean and safe living environment for residents.
- Communicate effectively with residents, families, and other team members.
- Follow all community policies and procedures to ensure the safety and well-being of residents.
Qualifications:
- High school diploma or equivalent required.
- Previous experience in a caregiving or healthcare role preferred.
- Licensed Nursing Assistant (LNA) certification is a plus.
- Strong interpersonal and communication skills.
- Compassionate, patient, and caring demeanor.
- Ability to work independently and as part of a team.
- Must be able to pass a background check

Position: Part-time Dining Room Server needed for Wednesday dinner shift and Friday lunch shift.
Job Responsibilities:
- Serve meals to residents in our dining room.
- Maintain cleanliness in the dining room and kitchen service areas.
- Work positively as part of a team to provide outstanding food and service to residents and employees.
